I would say waterjet or plasma cutter but for a product like that would require a cnc so therefore a compatible data file and then someone who has the machines and willing to do the work

Yes if you provide them with a CAD file. Depending on grade and thickness of metal it could be waterjet or laser cut.

Yes, to answer your question. Most Lazer shops have the abilityto scan a piece of material like that. If you need a scale up or scale down to be able to do it after the scan is made. Or if you’re able to locate whoever originally did that and they give you a DFX file of the project. you should be able to send it anywhere and get it cut out. To play such as Send cut send
