This is my fill setup and I'm leaking at the tank connection, is the problem that white washer and where can I find a replacement? Thanks Steve.

I suppose it’s possible there’s a nick in the seat inside your tank valve, so you might take a peak into it with a flashlight. If it’s nicked you would need a new valve. Hopefully it’s just the teflon nipple is damaged, although what’s able to be seen in your photo, it looks pretty good. But the photo doesn’t show 360° of nipple.
Hopefully Brancato sells replacement teflon nipple because his replacement nut and nipple is $89. I saw one that “looks” identical on ebay for $26.

Probably is the culprit. I use something similar but mine is just a compression fitting, no white seal and it will leak if you don’t cinch it down pretty well. Have you tried tightening it a bit more ?

I had ordered some from Amazon. Well, I think it was Amazon. I believe they are a 010 Teflon.

Your seal looks pretty good. Are you sure it's not leaking from the valve stem? Maybe you already soaped the hand tight fitting and know for sure.

Be sure to bubble test the assembled fill set and tank valve with soapy water so that you definitely know where the leak is originating from.

Me replaced that white oring with B90 and use it that way for years until the main scba valve orings disintegrated. Then replaced that valve with DIN, the worst was filling it back from zero.
