Tank / Gauge Failure

You didn't have the valve tightened up enough. The pressures these tanks hold , just the slightest small void will blow the o-ring at the neck. I use 90 durometer o-rings when replacing the valve , and finish tightening the connection with a rubber mallet. What size tank is that ? I have extra 0-rings for the larger tanks with the 7/8 -14 unf valve. If so, pm me and I'll send you a couple.

I personally have never seen a square cross section O ring used on a tank valve. But that doesn’t mean some tanks do use them.

Round buna 90 . #212 , O - ring. Better make sure the valve is snugged down tight. Any small void will create problems.................I know !

I think I didn’t tighten it down enough even though I did hand tight / snug.

The first pic what looks like metal is actually frost. Initially I thought it was a hole but when it came to room temp it disappeared so obviously frost build up from leak. O ring definitely blew out.
